All persons intending to attend the Conference must complete a Registration Form.  Please read the below registration information before you complete the Registration Form.  Should you experience any difficulties please contact the Conference Managers via email at melanoma2016@arinex.com.au

Registration fees listed above are in Australian Dollars (A$) and inclusive of goods and services tax (GST).

Registration Terms and Conditions

The 2 day and Nurse registration fee entitles all delegates to the following:

  • All sessions
  • Satchel: program booklet and list of participants
  • Social functions: Welcome Reception
  • Catering: Morning and afternoon teas, Lunches

 

Student Registration
A student is defined as a holder of a student identification card from a recognized tertiary or secondary educational institution or international student card. Students must be studying full time to qualify for the discounted rate. A copy of your student identification card must be sent to the Conference Managers.
A trainee is defined as a someone who is enrolled in a post graduate degree, medical school or professional training program at an educational institution. Evidence that you are enrolled in an educational institution must be sent to the Conference Managers.

Student and trainee registrants are entitled to the following:

  • All sessions
  • Satchel: Program Booklet and list of participants
  • Social Functions: Welcome Reception
  • Catering: Morning and afternoon teas

Registration confirmation

Your completed registration and successful payment will be acknowledged via email with confirmation of your requirements according to your registration form.  Your registration will only be processed or confirmed if payment does accompany your form

Name badge

Each member of the conference will receive a name badge upon registration. The badge will be your official pass and must be clearly visible at all times to obtain entry to all sessions and to social functions. The Conference Managers expect appropriate behaviour from attendees and reserve the right to revoke the badge and privileges of anyone behaving in an inappropriate manner.

Certificate of attendance

Your pre-arrival letter will include any further advice necessary prior to your arrival at the conference. Please see the conference registration staff onsite to claim your certificate of attendance.

Credit Card

Credit card is the preferred method of payment. Accepted credit cards are MasterCard, Visa and American Express. Please note all transaction by credit card will appear on your statement as payment to ‘Conference by Arinex Pty Ltd’.

Cheque / Money Order

All payments by cheque incur a $10 processing fee and will only be accepted up until 5 August 2016. Cheques are to be made payable to Australasian Melanoma Conference 2016.Your name and full address should be typed or printed clearly on the back of the cheque. Cheque payments from overseas must be a bank cheque in Australian Dollars drawn on an Australian Bank. No other currency will be accepted.

Cancellations must be notified in writing to the Conference Managers melanoma2016@arinex.com.au.

Registration Fees: Before 28 July 2016 will incur an A$150.00 cancellation fee. Before 16 September 2016 will receive a 50% refund of fees paid. Any cancellations on or after the 17 September 2016 will receive no refund.

Accommodation: A minimum one night’s deposit must be paid or credit card details given at time of booking to guarantee reservation. One nights deposit is non-refundable at 23 September 2016.

Social: Payments are non-refundable at 17 September 2016.

The program is correct at the time of publishing.  The organisers reserve the right to delete, modify or alter items from the program or to delete, modify or alter any aspect of the Conference timetabling and delivery at their sole discretion and without notice.  Neither the host organisation(s) nor the Conference Managers will accept any liability for any loss or inconvenience caused to any party consequent to such changes.

The Conference Managers reserve the right to use any recordings whether audio or video and/or any photographs taken at any stage during the Conference for promotional, advertising or reporting of the Conference and/or its content at their sole discretion.
